Gentamicin

Gentamicin dosing

Do NOT use if previous vestibular or auditory toxicity or serious hypersensitivity (rare) due to aminoglycosides

Severe sepsis or septic shock: A single gentamicin dose of 7 mg/kg. Subsequent dosing is adjusted in accordance with renal function (calculated creatinine clearance).

Severe sepsis: 7 mg/kg up to 640 mg (sepsis syndrome)

No evidence of severe sepsis: initial dose based on age, and subsequent dosing determined by renal function (calculated creatinine clearance) (see below)

Obese patients: Ideal body weight for aminoglycoside dose calculation, however do not delay FIRST DOSE if this is not available. Cap dose at 640mg

Empiric therapy: Use for a maximum of 48 hours as pending outcome of investigation.

NO levels required for empiric dosing (up to 3 doses)
